Preparing For Your Appointment

Because appointments can be brief, and because there's often multiple issues, it is best to be well prepared for your appointment. Here's some information to help you get the most from your appointment.

  • Make sure to write down your appointment time, if you are unable to come for some reason please notify us.
  • If you are a new patient, please print the history form by clicking DOWNLOADS link above, write down all your medical history and current medication-as well as any vitamins or supplements that you're taking (bring actual medications with you) – this will save a lot of time that we can focus on the actual issue for which you are being seen.
  • Write down any symptoms you're experiencing, including any that may seem unrelated to the reason for which you scheduled the appointment.
  • Write down key personal information, including any major stresses or recent life changes.
